First day Mail In Ballot Applications are accepted by Cook County Clerk. 

Applications can be requested from:

• City Clerk’s Office during business hours or call (847) 391-5311; or
• Call Cook County Clerk’s Office at (312) 603-0946 to request an application by mail; or
• Download an application from www.cookcountyclerk/elections.com; or
• Pick one up at Cook County Clerk's downtown Chicago office and five suburban courthouse mini-centers.
